Complete one registration for each athlete. Middle school tuition is $1,000; high school tuition is $1,500. $200 one-time fee for new rowers. Additional fees for uniform, rowing shoes, seat pad and regatta fees are not included in your fall registration.

By checking this box, I acknowledge that rowing and related activities—including practices, training, equipment handling, transportation, regattas, and activities on or near the water—may be hazardous and can result in property damage, serious injury, paralysis, or death. I authorize the athlete to participate and travel with Sarasota Scullers and authorize emergency medical treatment when a parent or guardian cannot be reached. I agree to follow coaches’ safety instructions and notify Sarasota Scullers of unsafe conditions. In consideration of participation, I release and hold harmless Sarasota Scullers Youth Rowing Program and its directors, officers, coaches, employees, volunteers, agents, sponsors, chaperones, and facility owners or operators from claims, damages, costs, and expenses arising from participation, except to the extent prohibited by law.

By checking this box, I authorize Sarasota Scullers to share the athlete’s health and emergency information with coaches, staff, volunteers, medical personnel, and others directly involved in the athlete’s care or safety. I understand information may be redisclosed where permitted by law and that I may revoke this authorization in writing except to the extent already acted upon. I confirm the athlete and parent or guardian will follow the Sarasota Scullers code of conduct and program rules; submitted information may be stored and used to administer the program; tuition and fees are nonrefundable; and regatta fees are separate.

By checking this box, I attest that the athlete can remain afloat without assistance for at least five minutes and swim at least 25 yards without touching the bottom or using support. I understand Sarasota Scullers will not independently verify this before participation and that the parent or guardian is responsible for the accuracy of this certification. An athlete who cannot satisfy these requirements may participate in on-water activities only while wearing a properly fitted personal flotation device and with coach approval.
